Muscle loss, functional weakness, and chronic pain rarely arrive suddenly. The conventional path forward — physical therapy, exercise, time — is valuable, but it has limits. Is the Foundation Program appropriate after joint replacement surgery?+
Yes — once your surgeon has cleared you for physical activity, typically 6–12 weeks post-surgery. EMSCULPT NEO® applicators are placed on the muscle belly itself, away from the joint and any hardware. We review your surgical notes and implant details at your assessment. EMSCULPT NEO® does not use the same electromagnetic field as MRI, and is compatible with most orthopedic hardware — but we verify this for each patient individually before beginning. Many post-replacement patients find that EMSCULPT NEO® produces meaningful quad and hamstring rebuilding that significantly extends their physical therapy gains and allows a more complete functional recovery.
Can the Foundation Program help with back pain?+
For back pain with a muscular root — which describes a significant proportion of chronic lower back pain cases — yes, meaningfully. EMSCULPT NEO® rebuilds the deep spinal stabilizers (multifidus, erector spinae) and gluteal muscles that are the primary muscular support for the lumbar spine. EMSELLA® restores pelvic floor tone that directly reduces downward pressure on the lumbar structures. We do not treat diagnosed disc herniations, spinal stenosis, or other structural pathologies — but for chronic back pain driven by muscular weakness and instability, the Foundation Program addresses the root cause most other treatments cannot reach.
How is this different from physical therapy?+
Physical therapy and the Foundation Program are complementary, not competing. Physical therapy teaches the nervous system how to recruit muscles correctly — movement patterns, motor control, biomechanics. The Foundation Program rebuilds the muscle mass itself, at a depth and intensity that voluntary exercise cannot achieve. Patients who have plateaued in physical therapy often find that Foundation sessions break through that ceiling by giving the muscles the structural rebuilding they need to actually perform the movements therapy is training. We actively encourage patients to continue working with their physical therapist alongside the Foundation Program, and we communicate with other providers at your request.
I'm a man. Is this program for me?+
Absolutely. The Foundation Program was built for any person dealing with functional strength loss — the patient populations we serve are roughly equal between men and women outside of the women's Core to Floor sub-program. Men dealing with chronic back pain, post-surgical quad or hamstring atrophy, pelvic floor dysfunction (including post-prostatectomy incontinence), core instability, or general muscle loss from age or injury are all excellent candidates. Many of our most dramatic functional outcomes have been in men who came in after joint replacement surgery and achieved a level of leg strength they hadn't had in years.
How many sessions will I need?+
This is determined at your assessment based on your specific deficit, goals, and timeline. EMSCULPT NEO® protocols for targeted muscle groups typically run 4–6 sessions spaced 2–7 days apart. EMSELLA® protocols for pelvic floor rehabilitation are typically 6 sessions, twice per week for 3 weeks. When both treatments are used in combination — as in a Foundation Program addressing back pain with core and pelvic floor components — the sessions can be scheduled concurrently or sequentially depending on your availability. We design the program around your life, not the other way around.
Will insurance cover any part of this?+
The Foundation Program treatments are generally considered elective and are not covered by standard health insurance. Some clients submit receipts to HSA or FSA accounts for reimbursement of therapeutic treatments — check with your plan administrator. We provide detailed, itemized documentation of all treatments received, which some patients use for out-of-network health savings reimbursement. We're transparent about pricing at your initial consultation — no surprises.
