EMSELLA® uses High Intensity Focused Electromagnetic (HIFEM) energy delivered through a specially designed chair to stimulate deep, supramaximal contractions throughout the entire pelvic floor — fully clothed, completely non-invasive, and with results that most patients begin to notice within the first few sessions. Establish the Foundation

EMSELLA® sessions begin the program — rebuilding pelvic floor strength and establishing the neuromuscular coordination that everything above it depends on. The foundation must be activated before the structure above it can be effectively loaded.

Why do both treatments together instead of separately?+
Because the core and pelvic floor are not two separate systems — they're four walls of one system. Treating only the pelvic floor leaves the abdominal walls weak, which increases downward pressure on the floor and limits how long results last. Treating only the core leaves the pelvic foundation compromised, which limits core stability and function. Together, they create a synergistic effect — each treatment making the other more effective and more durable — that neither produces on its own. The Core to Floor program is designed specifically around this anatomical reality.
How long does the full program take?+
Most Core to Floor programs are completed over approximately 5–6 weeks, with sessions scheduled 2–3 times per week depending on your chosen protocol. EMSELLA® sessions run 28 minutes; EMSCULPT NEO® sessions run 30 minutes. They can be scheduled on the same day (back to back) or on different days depending on your preference and schedule. Your exact program timeline is built around your availability at consultation — we design it to fit your life, not the other way around.
Is it uncomfortable to do both treatments in the same session?+
No — and most patients handle them well together. EMSELLA® is done seated, fully clothed, with rhythmic pelvic contractions. EMSCULPT NEO® involves the abdominal area with a different set of contractions. When scheduled back to back, patients typically find both manageable. Some prefer to space them on separate days, particularly in the first week as the body adapts to the sensations. We'll discuss what scheduling makes the most sense for you at your consultation and adjust as the program progresses.
I had a cesarean section. Can I still do Core to Floor?+
Yes — after appropriate healing. We require that your cesarean incision is fully healed and that you have received postpartum clearance from your OB or midwife, typically at your 6-week appointment. The EMSCULPT NEO® applicator is never placed directly over the incision site, and we assess scar healing carefully at your consultation before beginning core treatment. Many cesarean patients are excellent candidates for Core to Floor — the abdominal muscle disruption of surgery makes core rebuilding particularly important for their recovery.
I have diastasis recti. Will this help?+
EMSCULPT NEO® has shown promising results for diastasis recti — the separation of the linea alba that commonly occurs during pregnancy. By strengthening the transversus abdominis (the deep abdominal muscle that acts as a natural "corset" around the midline), EMSCULPT NEO® can help reduce the gap and restore abdominal integrity. Combining this with EMSELLA® addresses the pelvic floor component that is almost universally affected alongside diastasis recti. We discuss your specific situation at consultation, including the severity of any diastasis, before recommending the program.
How long do the results last?+
Because Core to Floor builds actual muscle mass and restores real neuromuscular function — rather than adding a substance that metabolizes or a toxin that wears off — results are meaningfully durable. Clinical data for EMSELLA® shows maintained quality-of-life improvements at 6 months post-treatment. EMSCULPT NEO® results can last a year or more with a healthy lifestyle. Should I talk to my doctor before starting?+
We strongly encourage it — and for patients with significant incontinence symptoms, we recommend discussing Core to Floor with your gynecologist or urogynecologist as part of a comprehensive care plan, not instead of one. Many of our Core to Floor patients come to us having already worked with a pelvic health physical therapist, and find that the program meaningfully extends and deepens the gains they've made. We're happy to coordinate with your other providers and provide documentation of your treatment for their records.
